Effects of Plan-Do-Check-Act Cycle Nursing Combined with Noise Management on Sleep and Clinical Outcomes in Children with Mycoplasma pneumoniae Pneumonia Complicated by Encephalitis

AI Summary
  • PDCA cycle nursing with noise management significantly improved sleep during hospitalisation: longer total sleep, fewer awakenings and fewer crying or startle episodes.
  • Associated with faster clinical recovery: shorter fever duration and reduced time to cough relief compared with routine nursing.
  • Lower overall complication rate (26% versus 48%, P = 0.023) and favourable changes in inflammatory and neurological biomarkers.

ABSTRACT

OBJECTIVE: This study aimed to examine the effects of Plan-Do-Check-Act (PDCA) cycle nursing combined with noise management on sleep indicators and clinical outcomes during hospitalization among children with Mycoplasma pneumoniae pneumonia (MPP) complicated by encephalitis.

METHODS: A retrospective analysis was conducted. A total of 100 children were classified into an observation group and a control group according to the nursing care actually received during hospitalization, with 50 children in each group. The control group received routine nursing care, whereas the observation group received PDCA cycle nursing combined with noise management in addition to routine nursing care. Baseline characteristics, sleep quality indicators, clinical improvement outcomes, biomarker indices, and the incidence of complications were compared between the two groups.

RESULTS: Baseline characteristics were generally comparable between the two groups. After nursing care, the observation group had longer total sleep duration [8.0 (7.0, 8.5) h versus 7.5 (7.0, 8.0) h, P = 0.019], fewer awakenings [2.0 (1.0, 3.0) versus 3.0 (2.0, 4.0), P = 0.005], and fewer crying/startle episodes [1.0 (0.0, 2.0) versus 2.0 (1.0, 3.0), P = 0.009]. Fever duration and time to cough relief were also shorter in the observation group ( P = 0.004 and P = 0.010). Several inflammatory, neurological injury-related, and cerebrospinal fluid indicators differed significantly after nursing care. The overall complication rate was lower in the observation group (26.00% versus 48.00%, P = 0.023).

CONCLUSION: Compared with routine nursing alone, the addition of PDCA cycle nursing combined with noise management was associated with greater improvement in sleep during hospitalization, faster relief of clinical symptoms, and a lower overall incidence of complications in children with MPP complicated by encephalitis, suggesting its potential value in optimizing nursing management for this population.
